Update: You guys called it! It was the parking brake. The material on them had disintegrated and was trapped on the inside of the rotor. I guess when it got "bunched up" just right, it made the grinding sound. So, removed the debris and cleaned the area. Everything looked intact minus at actual material from the shoes.

Chalk the brake shoes (just rub chalk over the surface of the brake shoe) Spin the wheel and apply the brake with the lever.
